Client Coaching Agreement

Client Responsibility Acknowledgement

By filling out this form you are consenting to this agreement for group coaching services. As a client of Grace Intel, you agree that you are responsible for creating your decisions and results.

The Coach is a Certified Wellness and Empowerment Coach and a member of the International Coach Federation with a background in Intellectual Wellness leadership, to work with education around empowerment skills and/or leadership, communications skills, career transition, facilitator training and public speaking.

Client Acknowledgment

Upon request, the coach may give specific advice or suggestions, however it is ultimately the client's responsibility for making his or her own decisions determining the best course of action. The coaching relationship is in no way to be construed as psychological counseling or any type of psychotherapy. If the client feels the need for professional counseling or therapy, it is the responsibility of the client to seek a licensed professional counselor.

Confidentiality

Our coaching relationship is completely confidential the coach recognizes that the client may disclose personal and private information the coach will not at any time share this information with a third party unless on the occasion information shared could be a threat to the client or of others determined by law. The coach will not at any time either directly or indirectly use any information for the coaches own benefit. The coach will not divulge that the coach and client are in a coaching relationship without express consent of the client. The client, of course is free to discuss the coaching relationship with anyone at any time. This confidentiality statement is intended to be as confidential as the applicable state or federal laws dash in the employer's own company policy parentheses if applicable) dash allows. The coaching relationship is not privileged and, as such, the coach says records regarding work with a client can be subpoenaed. Indemnification the client shall indemnify and hold the coach harmless from any loss or liability arising from actions taken or situations created because of the coaching relationship.

Fees

Grace Intel -prefers fee to be paid at the time of setting an appointment online or otherwise explained for other services such as group coaching. Coaching sessions typically run 50 minutes per session and cost is $80 per session. If extra time is needed at the time of the session, for $30 the client may have an additional 15 minutes to accommodate the need for more time.
